Step 1: Fill Out Health History Forms

• Fill out a New Patient Packet for family practice patients.
• If you are looking for mental health counseling, please fill out the Counseling Intake as well.
• Forms also available for pick-up inside the front door at Blue Mountain Clinic.
• If you are in need of abortion care, please call us at (406) 721-1646

Step 2: Submit your completed forms

• If you are unable to complete your packet online, you can:
  • Mail them to 610 N. California St, Missoula, MT 59802
  • Drop them off at BMC’s location. There is a secure dropbox located in the front entryway.
  • Fax them to 406.543.9890
  • Scan and email them to [email protected]

Step 3: BMC will call you to schedule

• We will contact you via phone to schedule your first appointment. If we are unable to match you with a provider, you will be notified via mail.
