• If you need to have your records forwarded from another medical provider, you may fill out our release of records ahead of time.

Step 1: Fill Out Health History Forms

• Fill out a New Patient Packet for family practice patients.
• Forms also available for pick-up inside the front door at Blue Mountain Clinic.
• If you are in need of abortion care, please call us at (406) 721-1646.
• NOTE: At this time, Blue Mountain Clinic is at capacity and unable to accept new patients seeking treatment for ADHD or chronic pain care that requires controlled substances, including stimulant or narcotic medications.

Step 2: Submit Your Completed Forms If you are unable to complete your packet online, you can:

• Mail them to 610 N. California St. Missoula, MT 59802
• Drop them off at BMC's location. There is a secure dropbox located in the front entryway.
• Scan and email them to [email protected].

Step 3: BMC Will Call You To Schedule We will contact you via phone to schedule your first appointment.
