If you are sick, have a bug bite, or are experiencing a myriad of other urgent issues that may be taken care of quickly, call the clinic and we will work you in. This may mean some waiting time in the lobby, but as soon as the next provider is available we will take care of you.

 

If you are a new patient, please have ready the following information available to give to the front office staff over the phone:

  • Insurance information - May require a co-pay at the time of visit
  • Current phone number - We confirm appointments the night before unless you specify otherwise
  • Specific reason for visit - If you need to schedule an abortion let the front office staff know and they will route your call appropriately

 

If you ever experience an emergency call 911, or go directly to the emergency room. On-call clinicians cannot schedule, refill prescriptions or diagnose over the phone.  These requests will be addressed during normal business hours. Payment is expected at the time of service.

 

If you are moving, or transferring your care to a different facility, please feel free to download our Release of Records form and our team in medical records will get them to the place of your choosing.

We offer a 20% discount on office visit fees to uninsured patients who pay cash in full at the time of service. Blue Mountain Clinic does not have a government funded sliding scale payment system and we are not a free clinic.  However, we will work with individuals as much as possible to facilitate access to care or referrals to other sources.
